Diabetes Prevention and Management Program

The Springs Medical Diabetes Prevention and Management Program provides a friendly and informal opportunity to learn about Type 2 Diabetes at a series of three 1.5hr workshops. The workshops are run by a Credentialled Diabetes Nurse Educator and Certified Health Coach and it is for anyone at risk of developing T2DM and those who have been diagnosed with Pre-Diabetes or Type 2 Diabetes.
In the workshops we explain what Diabetes is and how what we eat, how we move our bodies and how our emotional health relate to Diabetes. We provide lots of opportunities for questions and discussion with the benefit of connecting and learning from one another.
The program also includes one on one consultation with a Diabetes Nurse Educator where you can ask specific questions related to your health situation*. To wrap up the program a one on one follow up session with the Health Coach aims to help you consolidate and implement your learnings.
This program is funded by the West Victorian PHN in partnership with Maryborough District Health Service. There is no fee for this program.
*You may be referral to Central Highlands Rural Health to access one on one Diabetes Education.
