Falls & Balance Clinic


The Springs Medical Falls and Balance Clinic has been created to help patients that for a variety of different reasons, are at risk of falling or lack confidence in their balance. The program is designed and delivered by Accredited Exercise Physiologist Jake Dwyer and aims to reduce participants risk and rate of falls and hospitalisation, and to improve confidence and quality of life by maintaining independence and mobility.

We offer a 4 week exercise program which includes an initial assessment and two 30 minute exercise sessions per week for 4 weeks. The exercise sessions are held at Springs Medical Daylesford with a maximum of 5 per group. During the sessions the Exercise Physiologist will guide and support you through a series of predominantly low-intensity bodyweight exercises focusing on dynamic balance and gait. The Exercise Physiologist will choose exercises at a level appropriate to you so you progress in safe manner. You will also be educated on the benefits of regular and ongoing balance retraining and how the exercises work. The 4 week program is $72+GST and include 30 minute assessment, two 30 minutes exercise sessions each week for 4 weeks plus educational resources and discharge support. Download the patient information sheet here.

How to join the program

  • Springs GP Referral – discuss the program with your GP who can complete a referral to the program for you.

  • Self Referral – feel free to book a 30 minute Falls & Balance Assessment through our reception or online. Please complete this pre-registration questionnaire and bring it to your assessment. Download registration form here.

  • External Referrals – we accept referrals from non Springs Medical patients. Please ask your GP to complete and return this referral form for acceptance into the program. Please call our Program Coordinator on 0488 313 151 to discuss your involvement in the program.
