Health coaching is a personalized approach to helping patients achieve their wellness goals through guidance, support, and accountability. Health coaching emphasizes a proactive and positive approach, centering the patient as the expert in their own health and empowers patients to make sustainable lifestyle changes.
Your health coach will not tell you what to do, but will collaborate with you to identify your health goals, understand your strengths and what works well for you and navigate the obstacles that may come up. Whether it’s losing weight, managing stress, sleep, improving nutrition, or increasing physical activity, having more fun, your coach will help you set realistic and achievable goals and offer practical strategies and tools to overcome challenges.
What is involved?
Firstly, you will get to know your Health Coach and they will get to know you. You will have an in-depth discussion about what is important to you, your strengths, your values, your supports and your struggles. The most effective way to work with your coach is to commit to 12 sessions over 12 weeks so you can really build some momentum and growth. However, we know that may not work for everyone so you can discuss the frequency of sessions with your coach and work out a plan that works for you.
You will then start painting a picture of what your best health looks like, a vision of where you want to be. From there you will knuckle down into the details of what you specifically want to do to get there, design your goals and in each session review how you are getting on. You will be encouraged to think about what you are learning to help you design and build your next step.
The Springs Medical Health Coaching Program is funded by West Victorian PHN and delivered in partnership with Maryborough District Health Service. For concession card holders there is no fee for participating in the program. There is a small fee for non-concession card holders.
