Additional Covid-19 vaccine appointments for people aged 50 and above are available at Springs Medical Daylesford this weekend.

Please book online where possible as we are experiencing a high demand of calls. We are unable to move existing Covid-19 vaccine bookings forward – if you do want to change your appointment, please do so.

Patients wanting to source other vaccinations hubs (ie aged under 50) - Book your vaccine appointment | Coronavirus Victoria or call the Coronavirus hotline – 1800 675 398 There are no planned changes to GP appointments during the 7 day lockdown period. 

We will contact patients if there are any updates. Please remember, follow Department of Health guidelines – wear your mask, maintain social distancing, wash your hands and if you have any symptoms of Covid-19, please get tested and isolate.

SPRINGS MEDICAL COVID-19 VACCINE WAITING LIST FOR THE PHASE 1B IS NOW AVAILABLE.

 

PHASE 1b ELIGIBILITY

  • Elderly adults aged 80 years and over

  • Elderly adults aged 70-79 years

  • Other health care workers

  • Begin vaccinating Aboriginal and Torres Strait Islander adults

  • Adults with a specified medical condition

    *People will need to provide proof of these conditions to demonstrate their eligibility for vaccination via My health record, a health professional referral if required or a declaration form.

  • Adults with a disability who have a specified underlying medical condition

  • Critical and high risk workers including defence, police, fire, emergency services and meat processing

    * Workers will need to provide proof of occupation to demonstrate their eligibility.

  • Once you have determined that you are eligible, please complete the consent form and bring it with you to your appointment. DO NOT send your form to our clinic.

COVID-19 SYMPTOM CHECKER FROM HEALTH DIRECT

Our team at SM will continue to ask some routine questions in relation to your recent travel (as well as any current cold or flu symptoms) as part of our screening process to help keep everyone safe.

Your response to these questions will help us determine whether a face to face or telehealth consultation is the most appropriate for you.

***COMMUNITY ANNOUNCEMENT***

To ensure the safety of our community, Springs Medical is implementing preventative measures including our phone consultation model to support patients experiencing symptoms of cold and flu.

Rather than attending our Clinics, our reception will make a phone appointment for you. During the phone consultation, your GP will discuss your symptoms, ask questions about your recent travel history and provide a plan for your treatment. Medical certificates can be prepared and left for collection – your GP can discuss this further with you.

If your GP feels you should be tested for coronavirus, they may advise you to attend Ballarat Base Hospital for testing.
Please DO NOT ATTEND Springs Medical Clinics if you are experiencing symptoms of the cold and flu.
