Practice Coordinator

Level 8 (Health Professionals and Support Services Award 2010)

The Practice Coordinator leads and oversees the daily functions of the administration and reception team providing leadership and coaching to facilitate all members to achieve their maximum potential in their respective roles.

The role will also work as a member of the Springs Medical Leadership Team comprising the General Manager, Finance Manager and Clinical Team Leaders to ensure all clinical staff, associates and students are resourced and supported on a daily basis within a well-defined clinical governance and administrative framework.
• The role will also function as the OH&S Coordinator for Springs Medical.

Clinical Team Leader and Primary Care Nurse (RN) – permanent part time

The role will provide a senior leadership and strategic management focus and will work in close collaboration with the General Manager and SM Directors, Practice Coordinator, and Clinical Acute Care, Clinical Wellness Coordinators, Diabetes Educator, Mental Health Nurse, Clinical Admin Support, SIS program Coodinator and/or other programs and services as required as part of a larger multi-disciplinary team whose focus is on improving health outcomes for the local community. The role will ensure coordination between programs and the multidisciplinary team and support continuity of care. The role will provide mentoring, support safety and quality, continuous quality improvement, effective business development, program innovation.
Therefore, participation in the rostered nurse shifts, including Saturday mornings or late week day clinics, working with the on-call doctor especially at the Daylesford clinic may be required including to back up the nurse led clinics from time to time. The role may be required to deputise for the General Manager from time to time.
This position will support innovative and contemporary practice and medical and nursing students and GP Registrars during clinical placement.
In addition, the role will assist in the provision, monitoring and review of care to patients including clinical risk management to ensure quality standards are achieved and maintained and all care is delivered to a level that meet and aims to exceed RACGP standards.

Primary Care Nurse Registered Nurse (Div 1)

The role will provide a senior leadership focus on Chronic Disease Nursing that provide supportive clinical care within the context of a primary health care setting and as part of a larger multi-disciplinary team whose focus is on improving health outcomes for the local community.

This position will also support all areas of primary care nursing rotating across triage, procedures as well as chronic disease management. Participation in the rostered Saturday morning shifts working with the on call doctor especially at the Daylesford clinic may be required including to backing up the nurse led clinics from time to time. The role may require flexibility to expand chronic disease management focus to Trentham campus.

In addition the role will assist in the provision, monitoring and review of care to patients to ensure quality standards are achieved and maintained and all care is delivered to a level that exceeds standards.
