The Hepburn Shire SIS program is available for eligible patients living in Daylesford, Trentham, Creswick and the surrounding areas.

Please make an appointment with your GP to assess eligibility and for referral to SIS Wellness Coordinator.

Jack Clark

Jack holds a Bachelor’s in Exercise Science with a Master’s in Exercise Physiology from La Trobe University. He has a keen interest in exercise and education based cardiac and pulmonary condition management and rehabilitation.

Jack’s other area’s of interest:

• Management of cancer and treatment related side effects
• Musculoskeletal rehabilitation
• Functional-based exercise and rehabilitation

Outside of Exercise Physiology, Jack is also a keen footballer and enjoys all aspects of health and fitness as well as spending time with his friends and family.


All SIS participants will need to provide proof of Covid-19 vaccination prior to confirmation of their first class. We ask all participants to use our QR code or sign in sheet upon arrival.
